Please note that this form is only for resetting your password if you have access to the Hospital Referral Portal. Regular website users should reset their password via the normal method here.
To reset your password, please follow the instructions in the form below and enter the relevant information. You have 3 attempts before your account is locked temporarily.
You will need your current username that you normally use to log in to the referral portal as well as your 8 character authorisation code.
If you have not been asked to create the authorisation code or if you have forgotten the code or username, please contact patientchoicedelivery@nhs.net.
