Lymphoedema Sign Up form

This sign-up page is for nurses and clinicians/specialists that handle prescriptions for lymphoedema patients. It enables your clinic to sign up to the Patient Choice Home Delivery Service.

As a dispensing appliance contractor (DAC) we aim to deliver the best deal for patients and the NHS. On this page you can find both our sign-up form and a short video on why nurses are so positive about our approach and the difference it makes to them and their patients.

Once we receive this completed form, our Customer Experience team will contact you immediately to talk you through the process.
